Manejo da dor orofacial através do tratamento com acupuntura: relato de um caso

Resumo

As disfunções temporomandibulares (DTM) atingem de 5 a 12% da população adulta. Existem várias terapias propostas para seu manejo, sendo que as mais indicadas são as reversíveis e não invasivas; dentre estas, a acupuntura pode ser a de primeira escolha devido ao seu alto de índice de sucesso. A acupuntura é uma terapia milenar e trata de equilibrar o fluxo energético dentro de canais (meridianos) que circulam pelo corpo humano. Na medida em que a dor é provocada especialmente por bloqueio deste fluxo energético, o uso da acupuntura nestes casos se faz apropriado. O presente trabalho descreve o caso clínico da paciente SCDR, 38 anos, cuja queixa principal consistia de dor generalizada na região da cabeça, da articulação temporomandibular e da nuca, incluindo zumbido no ouvido e enxaqueca diária. Após realização da anamnese, além do diagnóstico estabelecido segundo preceitos ocidentais, elaborou-se o diagnóstico segundo os critérios da Medicina Tradicional Chinesa, sendo o tratamento instaurado de acordo com o desequilíbrio energético da paciente. O tratamento com a acupuntura foi utilizado em oito sessões semanais, sendo que a paciente relatou ter observado a regressão da dor e a remissão dos sintomas imediatamente ao término da primeira intervenção.

Abstract

Temporomandibular dysfunctions (TMD) affect from 5 to 12% of the adult population. Several therapies have been proposed for TMD management, the most indicated being the reversible and non invasive types, among them, acupuncture could be the first choice due to its high success rate. Acupuncture is an ancient millenarian therapy and balances the energy flow within the meridians that circulate throughout the human body, and as pain is caused especially by the blockage of this energy flow, the use of acupuncture is appropriate in these cases. This article describes the clinical case of a 38-year old patient, SCDR, whose chief complaint was generalized pain in the region of the head, temporomandibular joint and nucha (nape of the neck), including buzzing in the ears and daily migraine headache.   After anamnesis, in addition to the diagnosis established according to Western precepts, a diagnosis was prepared according to the criteria of Traditional Chinese Medicine, and treatment was instituted in accordance with the patient’s energy imbalance. Acupuncture treatment was used in twenty-two weekly sessions, and the patient reported having observed regression of pain and remission of symptoms immediately after the conclusion of the first intervention.
