Evaluation of the functional changes associate to the adaptation period in complete denture wearers

Resumo

A fase de adaptação funcional que ocorre nos pacientes após a instalação de próteses totais (PTs) é uma das mais críticas em relação à aceitação da prótese devido às dificuldades iniciais transitórias que o paciente enfrenta. O presente estudo teve como objetivo avaliar as possíveis alterações no padrão de movimentos mandibulares decorrentes da instalação de PTs em indivíduos edentados, durante o período de adaptação funcional das próteses. Foi empregado o sistema de diagnóstico cinesiográfico K6-I (Myotronics Research Inc., Seattle, WA) para a avaliação de 15 pacientes previamente tratados com confecção de PTs. Foram realizados registros da amplitude e da velocidade dos movimentos mandibulares de abertura e fechamento, do movimento da mandíbula entre a posição de repouso postural e a oclusão cêntrica e do movimento da mandíbula durante a mastigação e registro do limite de movimentos mandibulares de abertura e fechamento, lateralidade e protrusão. Foram realizadas seis sessões: I. no momento da instalação das PTs, após os ajustes necessários; II. 24 horas; III. 48 horas; IV. uma semana; V. 2 semanas; VI. 30 dias após instalação. Os dados obtidos foram tabulados e submetidos ao teste de Friedman (nível de significância 5%). Os resultados demonstraram que o padrão dos movimentos mandibulares pouco se altera em função do tempo de instalação estudado. Concluiu-se por meio de análise cinesiográfica, que a instalação de próteses totais não alterou o padrão de movimentos mandibulares em indivíduos edentados, durante um mês de acompanhamento.

Abstract

After complete denture insertion, edentulous patients usually present transitory difficulties. This is one of the most critical stages of prosthetic treatment and it is strongly related to the prostheses acceptance. The aim of this study was to evaluate potential changes in the mandibular movement pattern related to insertion of the complete denture during the functional adaptation period. The Kinesiograph instrument K6-I (Myotronics Research Inc., Seattle, WA) was used to evaluate fifteen edentulous patients that received conventional complete denture treatment. Recordings of opening and closure movement limits, movement velocity, postural rest position, chewing cycle and limits of eccentric movements were performed. Each patient was evaluated in six different stages: I. immediately after insertion of the new dentures; II. 24 hours; III. 48 hours; IV. 1 week; V. 2 weeks and VI. 30 days after insertion of the dentures. The results showed that there were no remarkable changes in the mandibular motion patterns after insertion of the new dentures. It was suggested that the problems related to the process of utilization of the new dentures are not strictly associated to the mandibular movement. Therefore, the insertion of the complete denture did not modify the mandibular movement pattern in edentulous patients, by means of kinesiographic study, during one month of accompaniment.
