Resposta dos tecidos periapicais de dentes de cães com necrose pulpar e reação periapical crônica ao tratamento endodôntico utilizando diferentes pastas obturadoras

Resumo

O objetivo da presente pesquisa foi avaliar, de maneira comparativa, a resposta dos tecidos periapicais de dentes de cães cujos canais foram obturados com diferentes pastas associadas ou não a curativo prévio. Para isso, foram utilizados 28 dentes de 3 cães com necrose pulpar e reação periapical crônica induzida, os quais tiveram os canais radiculares instrumentados sem o arrombamento dos ápices. Esses dentes foram divididos em 4 grupos: Grupo I - curativo de formocresol (10 minutos) + obturação com pasta de hidróxido de cálcio espessada com óxido de zinco; Grupo II - curativo de formocresol (10 minutos) + obturação com pasta de antibiótico; Grupo III - obturação com pasta de antibiótico; e Grupo IV - obturação com pasta de hidróxido de cálcio espessada com óxido de zinco. Tomadas radiográficas periapicais foram realizadas antes do procedimento operatório e após 180 dias, quando os animais foram sacrificados e as peças contendo dente e tecido ósseo adjacente foram removidas e processadas para análise histológica. Quando a pasta de antibiótico foi utilizada, observou-se redução na intensidade da resposta inflamatória, na reabsorção óssea e no espessamento do ligamento periodontal, tendo os resultados para os Grupos II e III sido estatisticamente semelhantes. Radiograficamente, apenas a pasta de antibiótico com e sem curativo prévio promoveu diminuição da área da lesão. De acordo com as condições experimentais, pôde-se concluir que a pasta de antibiótico associada ou não ao curativo de formocresol permitiu reparação do tecido periapical superior àquela observada com a pasta de hidróxido de cálcio.

Abstract

The aim of this investigation was to evaluate the response of periapical tissue after filling the contaminated root canals with different filling materials associated or not with previous application of a dressing agent. Twenty-eight teeth of three dogs with pulpal necrosis and induced chronic periapical lesion were divided into the following experimental groups according to the clinical procedures: Group I – formocresol dressing (10 minutes) + filling with calcium hydroxide paste mixed with zinc oxide; Group II – formocresol dressing (10 minutes) + filling with antibiotic paste; Group III – filling with antibiotic paste and Group IV – filling with calcium hydroxide paste mixed with zinc oxide. Periapical radiographs were taken and evaluated before the clinical procedures and 180 days after, when the animals were sacrificed. The teeth and adjacent bone tissue were surgically removed and processed for microscopic assessment. The statistical analysis of the scores determined for every histological events demonstrated no difference between the Groups II and III. However, only in Group II a significant decrease in the local inflammatory reaction, bone resorption and thickness of the periodontal ligament was observed when compared to the Groups I and IV. The radiographical analysis of the teeth demonstrated that in Groups II and III the periapical lesion area decreased. According to the experimental conditions, it was concluded that a more adequate periapical tissue repair occurred to the antibiotic paste combined or not with a formocresol dressing (Groups II and III) than to the calcium hydroxide saline paste (Groups I and IV).
