Avaliação Clínica e Microbiológica dos Efeitos da Irrigação de Gel de Metronidazol a 20% sobre a Raspagem e Alisamento Radicular em Pacientes com Doença Periodontal Crônica

Resumo

O estudo avaliou o efeito de irrigações com gel de Metronidazol a 20% como coadjuvante à raspagem e alisamento radicular (RAR) em pacientes com doença periodontal. Foram inclusos neste estudo cego seis pacientes com no mínimo três sítios em dentes unirradiculares com Profundidade de Sondagem entre 5-7 mm e BANA-positivos. Após RAR, os 32 sítios foram distribuídos aleatoriamente em um dos grupos: grupo – 1; RAR, grupo – 2; RAR + irrigação com gel placebo; grupo 3 – RAR + irrigação com gel de Metronidazol a 20%. As irrigações foram realizadas após a RAR inicial, 7, 14 e 21 dias após. Os parâmetros clínicos de Índice de Placa, Índice Gengival, Profundidade de Sondagem, Nível de Inserção Clínica e Sangramento à Sondagem foram realizados antes do tratamento, 60 e 90 dias após, e a avaliação microbiológica foi realizada com o Teste BANA antes do tratamento e 90 dias após. Os resultados demonstraram uma redução nos parâmetros clínicos sem diferença estatisticamente significante entre os três gru pos, com exceção da Profundidade de Sondagem, na qual o grupo 3 promoveu uma redução estatisticamente significante em relação aos grupos 1 e 2 aos 60 e 90 dias. Os grupos 2 e 3 promoveram uma redução estatisticamente maior nos sítios BANA-positivos que o grupo 1. Esses resultados sugerem que a associação de irrigação com gel de Metronidazol a 20% à RAR demonstrou efeitos clínicos e microbiológicos semelhantes à terapia de RAR iso lada.

Abstract

The study evaluate the effect of the irrigation with a 20% metronidazole gel as an adjunct to scaling and root planing (SRP) in patients with periodontal disease. Thirty two sites with PD 5-7 mm and BANA positive in 6 adult periodontitis subjects were selected. Sites were assigned to different treatment groups consisting of SRP only (group 1), SRP + irrigation with pacebo gel (group 2) or SRP + irrigation with 20% metronidazole gel (group 3). Irrigation was performed after inicial SRP and at 7, 14 and 21 days. Clinical measurements including PD, PI, GI, recession, BOP and PAL were performed at the selected sites at base line, 60 and 90 days while microbiological test was performed with BANA-test at base line and 90. The results demonstrated a reduction of the clinical parameters in all groups with out any difference between them, except in the Probing depth, where group 3 produced a significantly greater reduction than the groups 1 and 2 at both 60 and at 90 days. The groups 2 and 3 produced a more significant reduction in sites BANA positives than group 1. These results suggest that irrigation with 20% metronidazole gel should be considered promising method as an adjunct to SRP.
