Estudo comparativo da discrepância nas dimensões dentárias em pacientes portadores de diferentes deformidades esqueléticas

Resumo

As discrepâncias entre o tamanho mésio-distal dos dentes superiores e inferiores e seu efeito sobre a oclusão têm sido há muito tempo relatadas. Assim, o objetivo deste estudo foi avaliar as desarmonias do tamanho dentário, de acordo com o método de Bolton, em pacientes portadores de deformidades esqueléticas que necessitavam de cirurgia ortognática. A amostra foi composta de 90 pares de modelos de gesso, divididos igualmente entre os subgrupos de Classe I (controle), Classe II esquelética e Classe III esquelética. Foram obtidas as médias para as razões total e anterior e, então, avaliadas as diferenças entre os gêneros e tipos de más oclusões. Os resultados mostraram que as médias encontradas para a discrepância de tamanho dentário, em todos os subgrupos analisados, foram maiores do que as de Bolton. Os valores obtidos foram: Classe III (93,66), Classe I (92,51) e Classe II (92,06), não havendo diferença entre os gêneros. Quando comparados os grupos entre si, não houve diferença significante entre os subgrupos de más oclusões esqueléticas (Classe II e Classe III) e o grupo controle (Classe I), tanto para a razão total quanto para a anterior. Essa diferença só se mostrou significante quando comparados os subgrupos de Classe II e Classe III. Tais resultados mostram que, embora pacientes com deformidades esqueléticas apresentem maior discrepância de Bolton quando comparados a pacientes com oclusão normal, não foram encontradas diferenças estatisticamente significantes quanto à incidência de tal desproporção entre pacientes com más-oclusões de origem dentária em relação àquelas de origem esquelética.

Abstract

Mesiodistal size discrepancies of upper and lower teeth and its effect on occlusion have been related. The objective of this study was to evaluate the tooth size discrepancies according to the Bolton’s method, in patients with skeletal deformities requiring orthognatic surgery as part of their treatment. The sample was composed of 90 pairs of casts, equally divided among the subgroups of Class I (control), Class II skeletal and Class III skeletal malocclusion. Means for overall and anterior ratios were obtained for the whole sample, differences between gender and type of malocclusion were evaluated also. The results indicated that mean values of the teeth size discrepancies for all subgroups evaluated were bigger than Bolton index. Values found in our sample were: Class III (93.66), Class I (92.51) and Class II (92.06). There were no significant differences between gender. When all the groups were compared among each other, there were no significant differences among the subgroups of skeletal malocclusion (Class II and Class III) and the control group (Class I), for the overall and anterior arches. Significant difference was found among subgroup of Class II and subgroup of Class III. The results suggested that although patients with skeletal deformities had bigger Bolton’s discrepancy than patients with normal occlusion, there were no significant discrepancy differences among patients with dental malocclusion and patients with skeletal malocclusion.
