Variability on caries detection and decision making among undergraduates of Dentistry

Resumo

Mudanças no padrão de desenvolvimento da cárie dentária têm dificultado a sua detecção e determinação de sua extensão, bem como a elaboração de planos de tratamento adequados pelos profissionais. Apesar da importância do tema, pouco se sabe desta problemática durante o processo de formação dos alunos de cursos de Odontologia. Desta forma, o objetivo deste estudo foi avaliar a variabilidade acerca da detecção e decisão de tratamento para lesões cariosas em superfícies oclusais por meio do exame clínico-radiográfico bitewing de 20 dentes posteriores permanentes, sem cavitações aparentes, montados em manequim odontológico, entre graduandos do último ano de um curso de Odontologia de uma universidade pública do interior do Estado de São Paulo. A concordância interexaminador, obtida a partir da estatística Kappa, foi "moderada" para os critérios avaliados, ou seja, "detecção de cárie" (к = 0,54), "determinação da atividade de cárie" (к = 0,50) e "tomada de decisão clínica" (к = 0,52). O tratamento não-invasivo mais recomendado foi o "selante resinoso" (35,31%), enquanto "restauração com resina fotopolimerizável" foi o tratamento invasivo mais recomendado (71,1%), todavia houve diferenças marcantes sobre qual estratégia utilizar. Em face dos resultados obtidos, conclui-se que a implementação de estratégias de ensino/aprendizagem baseadas em treinamentos/calibrações constantes, visando minimizar estas variações, são necessárias, contribuindo assim, para a formação de um profissional dentro da filosofia de promoção de saúde.

Abstract

Changes in the pattern of dental caries development have been hindering their detection and extension determination, as well as the elaboration of appropriate treatment plans by the professionals. Although this theme is important, this problem during the process of the students’ graduation on Dentistry is little known. Anyway, the aim of this study was to evaluate the variability related to occlusal caries detection and their management by the visual-bitewing exam of 20 occlusal surfaces of permanent teeth, without cavities, mounted in a dental mannequin, by undergraduates of the final period of a course of Dentistry of a public university of the state of São Paulo, Brazil. The interexaminer agreement (Kappa statistics) was “moderated” for “caries detection” (к = 0.54), “caries activity” (к = 0.50) and “clinical decision making” (к = 0.52). “Sealant” (35.31%) was the most recommended non-invasive treatment, while “resin restoration” (71.1%) was the most recommended invasive treatment, differences on which strategy to use were found. Because wide discrepancy among the undergraduates related to occlusal caries detection and treatment was found, it was concluded that implementation of teaching/learning strategies based on constant training/calibration process are needed to minimize these variations and to contribute to a professional’s formation inside the philosophy of health promotion.
