Avaliação de confiança do CPITN em levantamentos epidemiológicos de condições periodontais

Reliability of the CPITN in epidemiological surveys of periodontal conditions

Resumo

Nos levantamentos epidemiológicos de condições periodontais e necessidades de tratamento utilizando o CPITN (Community Periodontal Index of Treatment Needs - Índice periodontal comunitário das necessidades de tratamento), a OMS sugere que apenas 10 dentes-indice, distribuidos em 6 sextantes (17,16; 11; 26,27; 37,36; 31; 46,47), sejam examinados. No presente estudo, procurou-se avaliar a confiança do CPITN por dois diferentes métodos de exame clínico: examinando-se todos os dentes presentes e apenas os dentes-índice sugeridos pela OMS. Um único examinador, previamente treinado, examinou duas vezes 80 indivíduos, de ambos os sexos, com idades entre 20 e 64 anos. No primeiro exame, todos os dentes presentes eram examinados e aos sextantes, atribuído o maior valor do CPITN observado. No segundo exame, somente os dentes-índice eram examinados e aos sextantes, atribuído o maior valor do CPITN observado. Os resultados mostraram que, para os sextantes posteriores esquerdos e direitos, tanto superiores como inferiores, houve excelente concordância entre os dois métodos de exame clínico, enquanto para os sextantes anteriores inferiores, o grau de concordância foi bom (Kappa = 0,78), e, para os sextantes anteriores superiores, o grau de concordância foi médio (Kappa = 0,62) .

Abstract

ln epidemiologieal surveys of periodontal conditions and treatment needs using the CPITN (Community Periodontal Index of Treatment Needs) the WHO suggests the examination of 10 index-teeth in 6 sextants (17,16; 11; 26,27; 37,36; 31; 46,47). The aim of this study was to assess the reliability of the CPITN in two different methods of elinieal examination: all present teeth and index teeth examination. Eighty individuals of both sexes aged 20 to 64 years were examined twice by one examiner, previously trained. ln the first examination all present teeth were examined and the higher observed CPITN score attributed to the sextant. ln the second examination only the index-teeth were examined and the higher observed CPITN score attributed to the sextant. The results showed an excelient degree of agreement between the two methods of examination for the maxillary and mandibular right and left sextants. The results also showed a good degree of agreement between the two methods of examination for the mandibular anterior sextants (Kappa = 0.78) and a regular degree of agreement between the two methods of examination for the maxillary anterior sextants (Kappa = 0.62).
